GENERAL SUMMARY

SALARY: $24.00/hour

The Medical Caseworker (Spanish-Speaking) is part of the Refugee Health Team within the Migration & Refugee Services program. This position is responsible for coordinating care for a caseload of Cuban, Haitian, and refugee clients. The caseworker will assist clients in connecting to health services and providers through scheduling appointments, assisting with transportation, communicating with clinics, and making referrals, among other tasks. This position ensures medical services are provided to the client within the guidelines specified by the various funding sources. The MC is expected to work in partnership with fellow caseworkers to implement a systematic and organized approach to provide medical case management services to clients. The MC will also be responsible for maintaining healthy relationships with outside partners, specifically those in the mental health and medical community.

EDUCATION and EXPERIENCE
Associates degree preferred or studies leading toward a degree.
At least three years of work experience preferably with refugees.

ESSENTIAL DUITES and TASKS
1.
      Demonstrates good communication skills to successfully interact with colleagues, clients, volunteers, and service providers in a multicultural environment and with individuals with limited English-speaking ability
2.      Schedule health assessments for new arrivals  
3.      Scheduling follow-up care, specialist visits, and vaccine appointments.  
4.      Coordinate care for any special medical needs, such as referral to specialists or consultation with medical providers. 
5.      Conduct weekly Medical Orientation for new clients. 
6.      Connect clients to primary care providers.
7.      Contact clients to inform them of appointments and other health related information.
8.      Transport clients or assist clients with finding transportation to medical appointments.
9.      Ensure that paperwork, documentation, and assigned reports are completed in a timely manner.
10.   Identify and resolve problems in a timely manner as part of a team.
11.  Assist in maintaining and checking case files for the Refugee Health Team.
12.  All other duties assigned by the Supervisor.
Education Requirements (Any)
Associates degree or 2 years of college

  • Must be able to communicate well verbally and in writing with colleagues, clients, other community personnel, and service providers
  • Must be able to work under pressure and handle stressful situations
  • Must be proficient in PC skills including Microsoft Office and Outlook
  • Must be able to maintain confidentiality of information
  • Must demonstrate self-confidence, initiative, flexibility, good supervisory skills, and a high degree of professionalism
