The Wellspring CSU Case Manager provides Case Management to adults with severe and persistent mental illnesses who are homeless and receiving services in the Crisis Stabilizaiton Unit. The ideal candidate will have a belief in recovery from mental illness and an enthusiasm for helping persons with disabilities, rooted in empathy and compassion and demonstrating a careful and caring attitude. The CM assists clients in gaining access to medical, social, educational, housing and other support services. The CM develops and facilitates pursuit of individualized recovery goals via assessment, connecting to resources and monitoring progress. The Wellspring CSU CM's duties include but are not limited to: assisting clients in pursuing housing options, supervision of clients' self-administered medications (SSAM), assisting clients with their engagement in physical and mental health and addiction treatment and ensuring uninterrupted provision of all services. The CSU CM models and assists clients with illness management and independent living skills and provides crisis intervention as needed. The CSU CM participates in monthly staff meetings and collaborates daily with Wellspring CSU clinical and program staff as well as other providers and sources of support. The CSU CM completes electronic documentation of services demonstrating medical necessity within 24 hours of client contact.

A personal vehicle is required as the CSU CM will be responsible for transporting clients to regular appointments.

Education Requirements (Any)
*BA/BS in psychology
social work or related field and at least one year post Bachelor's working full-time with adults with SMI
