The Wellspring HUD Case Manager provides Case Management to homeless adults with severe mental illnesses. The ideal candidate will have a belief in recovery from mental illness and an enthusiasm for helping persons with disabilities, rooted in empathy and compassion and demonstrating a careful and caring attitude. The HUD Case Manager assesses client's needs and goals and provides the supportive services necessary for the clients to obtain and maintain stable housing and maximize self-sufficiency and self-determination. The HUD Case Manager develops and facilitates pursuit of individualized recovery goals via assessment, connecting to resources and monitoring progress. The Wellspring HUD Case Manager's duties include but are not limited to: providing linkage and referrals to resources; providing coordination and communication with medical, employment and educaitonal programs; assisting and advocating in applying for and renewing mainstream benefits; providing supportive counseling, advocacy and crisis prevention/intervention services.

A personal vehicle is required as the HUD Case Manager will be responsible for transporting clients to regular appointments.

Education Requirements (Any)
BA or BS degree and 1 year SW experience
